"The Workforce Planning and Development Boards of Southwestern Ontario would like to evaluate how the COVID-19 crisis has affected the workforce in Southwestern Ontario. This survey asks a series of demographic questions and questions about how the crisis has affected you.
This survey should take about 5 to 10 minutes to complete.Your responses are completely anonymous. The Workforce Planning and Development Boards of Southwestern Ontario will maintain the responses confidential. Some questions towards the end of the survey are sensitive and may be a source of distress. If you are facing anxiety or stress as a result of the COVID-19 crisis, you can refer to trusted sources like theWorld Health Organization, or reach out to your local healthcare provider or public health authority. Your participation in this survey is voluntary and you can choose to exit the survey at any point.
